Key Responsibilities\n

• Conduct thorough psychosocial assessments by interviewing patients and their families, observing behaviors, and reviewing medical records to identify emotional, social, and environmental factors that may impede recovery.

• Provide counseling services aimed at addressing emotional well-being, supporting patients and families with adjustment issues, and promoting coping strategies as they face the challenges of rehabilitation.

• Assist patients in navigating community resources, including access to support groups, vocational rehabilitation services, and housing assistance, to foster their independence and post-rehabilitation success.

• Educate patients and families about available community resources and ongoing support services that will enhance their recovery and reintegration into the community.

Patient Population / Client Focus\n

Compensation and Benefits\n
