Key Responsibilities\n

• Conduct in-depth virtual psychosocial assessments to identify clients' strengths, needs, and mental health challenges.

• Develop, implement, and monitor personalized treatment plans utilizing evidence-based practices to ensure effective intervention.

• Provide virtual therapy sessions for individuals, families, and groups, employing various therapeutic modalities tailored to client needs.

• Work collaboratively with physicians, nurses, and other healthcare professionals to coordinate integrated care, ensuring a holistic approach to client wellness.

• Act as an advocate for clients, helping them to navigate community resources and access necessary support services.

• Maintain accurate documentation of client interactions, treatment progress, and outcome measurements in electronic health records (EHR) systems.

Patient Population / Client Focus\n

This role primarily involves working with clients in a hospice setting, frequently addressing complex emotional and psychological needs. Qualifications\n

• A Bachelor’s degree in Social Work (BSW) from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• A minimum of two (2) years of post-licensure experience in providing clinical social work services, ideally within a hospice or healthcare setting.

Preferred Experience\n

• Experience in hospice care or with terminally ill populations.

• History of providing group therapy and leading workshops for clients and families.

Compensation and Benefits\n
