Key Responsibilities\n

• Conduct ongoing assessments tailored to the needs of individual patients, ensuring a holistic understanding of their mental health journey.

• Engage in open communication with supervisors, direct care staff, family members, and other stakeholders about each individual’s progress, fostering a collaborative treatment approach.

• Prepare and provide a tentative schedule for appointments, home visits, and observations to monitor each patient’s progress effectively.
\n
• Coordinate home visits that encompass in-home observations, comprehensive chart reviews, staff interviews, and rigorous data analysis to address patients’ needs more efficiently.

• Ensure that the implementation of program plans is executed effectively and in alignment with best practices.
\n
• Actively participate in Interdisciplinary Team Meetings to evaluate patient progress and strategize further interventions as needed.
\n
• Document monthly and quarterly progress notes to track results and inform ongoing treatment strategies.
\n
• Organize training initiatives to ensure all staff members involved in the implementation of the program plan adhere to established protocols and documentation standards.
\n
• Provide follow-up interviews, home visits, and observations, ensuring continuous support tailored to each individual’s unique needs.
\n
• Maintain accurate and accessible records, ensuring that essential information is readily available for all relevant parties.
\n
• Act as a liaison with the broader public and the mental health community to foster relationships that support developmental goals.
\n
• Help design and refine procedures for creating training materials aimed at enhancing the quality of care provided to our patients.

Patient Population / Client Focus\n

You will work with a diverse range of patient populations, including both adults and youth dealing with various mental health challenges. This role allows you to tailor your expertise in areas such as family therapy, couples counseling, substance misuse counseling, or trauma-informed care to directly impact your patients' lives and facilitate their path to recovery.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is a requirement for this position.

• An active and unrestricted license as a Professional Counselor in the state of Arizona is necessary.
\n
• A minimum of 2 years of counseling experience is required, preferably in a clinical context.

• Specialization in family therapy, couples therapy, trauma, or substance misuse is highly desirable.
\n
• Strong understanding of legal and ethical standards in mental health practice.
\n
• Demonstrated excellent interpersonal and communication skills to build rapport with a diverse client base.

• Prior demonstrated effectiveness working collaboratively within a healthcare team.

• Familiarity with electronic medical records (EMR) software and associated technologies is essential for efficient documentation and coordination.

Preferred Experience\n
