Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients through in-depth interviews, observations, and analysis of medical records. This will enable you to identify emotional, social, and environmental factors impacting their recovery journey.

• Offer tailored counseling and emotional support to patients and families as they navigate adjustment issues related to their health challenges, helping them cope with change.

• Collaborate closely with the rehabilitation team—including occupational therapists, physical therapists, and speech-language pathologists—to develop and implement individualized treatment plans that prioritize patients' psychosocial needs.

• Facilitate access to community resources, including support groups, vocational rehabilitation, and housing assistance, ensuring each patient has the tools and support systems necessary for a successful recovery.

• Educate patients and their families about available community resources, post-rehabilitation services, and coping strategies to help maintain their well-being after discharge.

• Contribute to the discharge planning process, preparing patients for transitions by providing follow-up guidance and ensuring continuity of care.

The patients you will serve come from diverse backgrounds and face unique challenges, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders.
