Work Environment

The physical therapist will work primarily in patients' homes, ensuring they receive hands-on, personalized care in a comfortable and familiar setting. Our clinic operates Monday through Friday from 9 AM to 5 PM, allowing for a balanced work-life dynamic. With an average caseload of 7 patients per day, our therapists are equipped with tools and resources to make meaningful connections with each patient, thereby enhancing their overall therapeutic experience.

Key Responsibilities

• Conduct comprehensive evaluations of patients to identify their specific needs and goals.
• Develop tailored treatment plans utilizing advanced rehabilitative techniques such as manual therapy, therapeutic exercises, and patient education.
• Oversee the implementation of therapy programs while adapting strategies based on patients' progress and feedback.
• Monitor and document patients' progress, using evidence-based metrics and adjusting care plans accordingly to optimize outcomes.
• Provide education to patients and their families about recovery processes, exercises, and injury prevention.
• Maintain a high standard of patient confidentiality and adhere to all HIPAA regulations during home visits.

Patient Population / Client Focus

The therapist will primarily serve adult patients recovering from surgeries, injuries, or chronic conditions. We have a diverse patient demographic, including individuals healing from orthopedic surgeries, those managing chronic pain, and patients seeking to regain mobility after accidents or falls. The ability to build rapport and trust while addressing each patient's unique challenges is essential to ensuring a successful therapeutic relationship.

Qualifications

• Current licensure as a Physical Therapist in New York State is required.
• A Bachelor's degree in Physical Therapy or a related field is mandatory, while a Master's or Doctorate degree is preferred.
• A minimum of 2 years' experience in an outpatient therapy setting is required, showcasing proficiency in various therapeutic techniques and patient treatment modalities.
