Work Environment

The role is based onsite at a school setting in Rye, New York, providing a unique opportunity to engage with a diverse patient population, including students and their families. The work hours are structured from Monday to Friday, totaling 40 hours a week with occasional Saturday coverage, allowing for a balanced work-life dynamic while ensuring consistent patient support.

Key Responsibilities

• Manage the total healthcare needs of patients, addressing both physical and psychological issues to ensure holistic care.
• Utilize advanced diagnostic skills to identify normal and abnormal health findings, ensuring timely and accurate assessments.
• Develop and implement individualized treatment plans based on patient assessments, continuously reassessing and revising plans as necessary.
• Evaluate the patient’s reactions to prescribed medications and treatments, adjusting protocols to ensure optimal outcomes.
• Administer and prescribe immunizations according to established guidelines, emphasizing preventative healthcare.
• Establish seamless communication channels with physicians and other healthcare team members to orchestrate coordinated patient care.
• Maintain a detailed Problem/Medication list during each patient encounter to monitor health developments.
• Ensure thorough, accurate, and timely documentation of each patient interaction in accordance with industry standards.

Patient Population / Client Focus

As a Nurse Practitioner at Wide Stream Pain Solutions, you will primarily serve a diverse patient demographic, including school-aged children, adolescents, and their families. You will focus on cardiovascular health issues, providing care that emphasizes not just treatment, but education and prevention, fostering a healthier community.

Qualifications

• A Master’s degree in Nursing from an accredited Nurse Practitioner program.
• Current Registered Nurse (RN) license valid in New York State.
• Basic Life Support (BLS) Certification.

Preferred Experience

• A minimum of 2-3 years of clinical experience, with a preference for candidates who have worked in cardiology or similar healthcare settings.
