Key Responsibilities\n

• Triage both urgent and routine referrals from medical providers to ensure timely access to care.

• Collaborate with office staff and medical providers to effectively schedule patients for intake and follow-up sessions.

• Participate actively in care team huddles, communicating insights and updates on client progress with other health care professionals.

• Develop and implement individualized, client-centered treatment plans based on thorough assessments and ongoing evaluation of client needs.

• Manage a range of case management tasks including advocacy, medication monitoring, emergency services coordination, and liaising with insurance providers.

• Provide education, referrals, and consultations to clients and external agencies as necessary, creating a holistic support network.

Patient Population / Client Focus\n

You will be working with a varied patient population that includes individuals experiencing anxiety, depression, relationship issues, and trauma-related concerns. Qualifications\n

• A Master's degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• Current licensure as a Licensed Social Worker (LSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC) in Pennsylvania.

Compensation and Benefits\n

The position offers a competitive salary range of USD 90,000 to 100,000 annually, based on your caseload and experience.
