Key Responsibilities\n

• Conduct thorough, age, and culturally appropriate bio-psychosocial assessments to identify clients' mental health, substance abuse, and medical needs.

• Facilitate individual and group therapy sessions, tailoring approaches to the specific challenges faced by patients and their families in hospice settings.
\n
• Document all evaluations, treatment progress, and crisis interventions electronically, ensuring accurate and timely records that facilitate patient care continuity.
\n
• Identify and escalate patient needs to medical providers as appropriate, advocating for the best interests of clients and ensuring a well-rounded care approach.
\n
• Provide liaison services and social work support, connecting clients and their families to local resources, including medication assistance programs and community agencies.
\n
• Collaborate with Quality Improvement teams to develop, implement, and refine processes for data collection aimed at improving outcomes and service efficiency.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families navigating the emotional and psychosocial complexities associated with terminal illnesses and end-of-life care. You will provide compassionate support that respects the dignity of each client, offering tailored solutions that address their unique challenges in a hospice context.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.
\n
• Current and valid license as a Licensed Clinical Social Worker (LCSW) in the state of California.
\n
• Minimum of three (3) years post-licensure experience in a relevant mental health setting, with a strong understanding of hospice care principles.

• Proficient knowledge in counseling techniques, crisis intervention methodologies, and an ability to make independent judgments related to client care.

• Strong skills in group facilitation and a proactive approach to treatment planning and emergency interventions.

Preferred Experience\n

• Experience in hospice or palliative care settings, with familiarity of the unique challenges faced by these populations.

• Demonstrated success in building collaborative relationships within interdisciplinary teams.

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 115,000 to 130,000 annually. Additionally, we provide a comprehensive benefits package, including:
