Key Responsibilities\n

• Utilize your educational background and clinical expertise to conduct thorough psychosocial assessments and develop individualized treatment plans for hospice patients.

• Engage with clients using evidence-based therapeutic modalities such as solution-focused therapy, motivational interviewing, and behavioral activation to address their unique emotional and psychological challenges.

• Monitor therapeutic interventions closely, gathering feedback to evaluate the effectiveness of the treatment strategies employed.

• Collaborate with a multidisciplinary team including medical doctors, nurses, and psychologists to facilitate a holistic approach to patient care, ensuring continuity and integration of services.

• Maintain accurate documentation in compliance with health regulations and organizational policies while using electronic health record systems for efficient record management.

• Support patients in understanding and managing any psychotropic medications prescribed, with particular attention to adherence and side effects, enhancing treatment outcomes.

Patient Population / Client Focus\n

Our primary focus is on hospice patients who are experiencing emotional distress, anxiety, and complex grief related to end-of-life issues. You will work closely with individuals and their families, providing empathetic support and intervention that respects their diverse backgrounds, cultures, and belief systems.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current, valid, and unrestricted license to practice independently in mental health as a Licensed Clinical Social Worker (LCSW) in the state of Arizona.

• A minimum of one year of clinical experience as an independently licensed therapist, particularly in hospice, palliative care, or geriatric settings is preferred.
