Work Environment\n

This role is based remotely but collaborates closely with our Hospice Center team in Sacramento, California, ensuring that all staff adhere to the highest standards of care while working alongside practitioners in end-of-life care. You will use telehealth platforms and other digital tools to connect with clients, conduct assessments, and deliver services that meet regulatory standards. Key Responsibilities\n

• Conduct thorough evaluations and assessments of children with disabilities or developmental delays, utilizing a variety of standardized tools.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), collaborating with families to create actionable and effective plans.
\n
• Monitor and document client outcomes, ensuring alignment with established metrics while working as part of a multidisciplinary team.
\n
• Provide continuous support and guidance to families, empowering them to enhance their child’s development through education and evidence-based practices.
\n
• Utilize natural environment teaching and a coaching model, tailoring interventions to respect individual family dynamics and learning preferences.

• Ensure compliance with all relevant federal and state regulations, maintaining high standards of professional practice.

• Develop and maintain rapport with families, children, and team members, creating a nurturing and positive learning environment.

• Stay updated on best practices and participate in ongoing professional development opportunities.

Patient Population / Client Focus\n

Your primary focus will be working with infants and toddlers facing developmental challenges, providing them with the support they need to thrive. You will help facilitate a seamless transition between home and educational settings by addressing social, emotional, cognitive, and communication skills. This role requires a deep empathy and understanding of the unique pressures faced by families navigating these early developmental stages.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) by the Department of Public Health.

• A valid driver’s license and a clear understanding of the requirements involved in delivering social work services both in person and remotely.

• Strong organizational skills and the ability to manage time efficiently, especially in a remote setting.

• Excellent verbal and written communication abilities, facilitating effective collaboration with families and professional peers.

Compensation and Benefits\n
