Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to meet the age and cultural backgrounds of clients in a primary care setting.

• Deliver behavioral health services to individuals, couples, and families, encompassing counseling, intake assessments, diagnostic evaluations, treatment planning, crisis intervention, and referrals to substance use treatment programs.

• Provide ongoing support and education by facilitating group counseling sessions designed to address behavioral concerns and foster community engagement.
\n
• Document all clinical interactions accurately and timely in electronic health records, including progress notes and mental health evaluations, ensuring compliance with regulatory standards.

• Work collaboratively with Quality Improvement teams to gather and analyze data aimed at enhancing service delivery and client outcomes.
\n
• Serve as a liaison for clients seeking local resources and community agency support, including medication assistance programs, thereby promoting comprehensive care pathways.

Patient Population / Client Focus\n

You will primarily work with individuals grappling with substance use disorders as they transition through detoxification. This population often requires trauma-informed care and culturally sensitive approaches, making your expertise critical in delivering effective treatment. Qualifications\n

• Master’s degree in Social Work from an accredited institution is required.
