Registered Nurse (RN) - Home Health

Location: Mesa County, CO

Assignment Duration: This is a 13-week contract position.

Schedule: Flexible Monday-Friday day schedule with on-call rotation.

Job Description: SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This is an in-person home health position offering flexibility in how you structure your day, along with manageable caseloads and meaningful one-on-one care.

Responsibilities:

• Perform comprehensive patient assessments.
• Develop and update individualized plans of care.
• Provide skilled nursing care and appropriate interventions.
• Monitor patient progress and report changes to the care team.
• Educate patients and families on medications, treatments, and care plans.
• Coordinate care with LPNs, therapists, caregivers, and providers.
• Complete timely and accurate documentation in EMR.
• Participate in on-call rotation.

Required Experience / Certifications / Licensure:

• Active Registered Nurse (RN) License in the State of Colorado (or active NLC compact license).
• Valid driver's license, reliable transportation, and auto insurance.
• Strong assessment and critical thinking skills.
• Ability to work independently and manage a structured caseload.
• Clear, professional communication with patients and team members.
• Comfort with documentation and EMR systems (HCHB preferred).
• Home health experience preferred, but not required.
