Key Responsibilities\n

• Actively collaborate with healthcare professionals to devise and implement effective treatment plans that enhance patient well-being.

• Develop and coordinate comprehensive discharge plans, connecting patients with appropriate community resources to ensure continuity of care post-discharge.

• Deliver crisis intervention and support during critical health events, playing a key role in mental health stabilization for patients and their families.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
