Key Responsibilities

• Conduct comprehensive evaluations by reviewing patients' medical histories and performing specific assessments to create individualized treatment plans.
• Effectively communicate evaluation findings and treatment goals to patients and their families, engaging them in the goal-setting process for better outcomes.
• Implement treatment protocols with precision, continuously adapting techniques based on each patient's condition and the latest healthcare advancements.
• Collaborate closely with other healthcare professionals as necessary, recognizing changes in patients' conditions that may require modifications to care plans or additional consultations.
• Manage discharge planning and follow-up care effectively, ensuring accurate and timely documentation of patients' treatment experiences and progress through legible notes and summaries.
• Demonstrate strategic time management to ensure that productivity expectations are met, including maintaining an organized schedule and participating in clinical meetings.
