Responsibilities

• Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
• Collect relevant data, analyze assessment findings, and develop a care plan that prescribes interventions to achieve outcomes.
• Implement the plan, coordinate care delivery with other disciplines, and promote a safe, healing environment.
• Evaluate progress toward outcomes, adjust plans as needed, and participate in discharge planning to ensure continuity of care.
• Supervise and direct activities of various levels of assigned nursing staff; delegate appropriately and coordinate duties.
• Educate patients, families, and treatment team; provide clear health information and support learning for ongoing care needs.

Requirements

• Licensure: Current Registered Nurse (RN) license in the state of practice (California required).
• Certifications: CPR or Basic Life Support (BLS) required; HLFS (Hospital Fire and Life Safety) required; ACLS or HS-ACLS or RQI-ACLS required; PALS or HS-PALS or RQI-PALS required; NIH Stroke Scale (NIHSS) required.
• Experience: At least two years of recent nursing experience in the field; acute care experience preferred.
• Education: Bachelor’s Degree in Nursing (BSN) preferred.
• EMR: Recent EPIC experience required (within the last 12 months).
• Setting: Ability to work effectively in a home healthcare - hospice environment.
• Shift readiness: Ability to work night shifts (3x12 schedule).
