Key Responsibilities\n

• Provide one-on-one counseling sessions and group support to patients and their families, focusing on emotional adjustment, coping strategies, and overall mental health enhancement.

• Collaborate actively with the rehabilitation team of healthcare professionals, including physical and occupational therapists, to develop tailored treatment plans that address the unique psychosocial needs of each patient.

• Support patients in accessing vital community resources, such as support groups, vocational rehabilitation services, and housing assistance, ensuring a well-rounded approach to their post-rehabilitation lifestyle.

• Lead educational initiatives for patients and families about available community resources and post-rehabilitation services, enabling informed transitions and successful reintegration into daily life.

• Participate in discharge planning processes, helping to navigate transitions back to home care or other medical settings in accordance with the best practices.

Patient Population / Client Focus\n

Qualifications\n

• A Master's Degree in Social Work from an accredited program is mandatory.
