Work Environment

This position is situated in a home care setting based in Goldenrod, Florida, where you will work directly with clients in their own homes. Our commitment to a comfortable and flexible working environment allows you to engage with clients in a familiar setting, thus enhancing their rehabilitation experience. By working onsite in clients’ homes, you have the unique opportunity to assess their living environments, facilitate their recovery processes, and provide therapy that significantly improves their quality of life.

Key Responsibilities

• Design and implement individualized physical therapy treatment plans focused on preventing disability, alleviating pain, and enhancing functional mobility in clients.
• Directly oversee the activities of physical therapy assistants, ensuring effective execution of the care plan while maintaining compliance with regulatory standards.
• Conduct comprehensive evaluations of clients' functional levels through diagnostic testing and assessments, taking into account particular needs and environmental factors.
• Assess home environments for suitability regarding rehabilitation and provide actionable recommendations to clients and their family members.
• Administer therapeutic exercises, traction, and various physical modalities, while motivating clients through individualized exercise and rehabilitation plans.
• Evaluate, fit, and make recommendations on orthotic and prosthetic devices, ensuring optimal functioning and comfort for clients.
• Maintain accurate clinical documentation, including initial evaluations, progress notes, and discharge summaries, compliant with OASIS guidelines and company policies.
• Participate in interdisciplinary team meetings, contributing insights regarding clients’ progress and collaboratively planning objectives for comprehensive care.
• Facilitate communication and collaboration with clients’ families, providing education on home care practices related to physical therapy.
• Complete all documentation and therapy orders in a timely manner, in alignment with branch protocols.
• Supervise and support Physical Therapy Assistants in delivering care, ensuring adherence to best practices and applicable legal guidelines.
• Engage in ongoing education and mentorship with team members to foster a culture of excellence in physical therapy.

Patient Population / Client Focus

Our clients range from post-operative patients requiring physical rehabilitation to individuals with chronic conditions affecting mobility. You will work closely with seniors, individuals recovering from surgery or injury, and those coping with long-term health issues. We focus on delivering compassionate care to each patient, adjusting approaches based on their unique circumstances to maximize their independence and functionality.

Qualifications

• A Bachelor’s degree in Physical Therapy from an accredited program is required.
• A minimum of one year of hands-on experience in a physical therapy role, preferably in home care or acute care settings.
• Valid and current physical therapy licensure in the state of Florida.
• Current CPR certification.
• Exemplary interpersonal and communication skills for effective patient and family engagement.
• Strong commitment to clinical excellence, patient-centered care, and lifelong learning.
• Ability to work collaboratively within an interdisciplinary team while inspiring patients and families through positive interactions.

Preferred Experience

• Experience using electronic health record (EHR) systems, with the capability to manage documentation and care plans effectively.

Compensation and Benefits

The annual salary range for this position is between USD 100,000 and 120,000, commensurate with experience and education.
