Key Responsibilities\n

• Assist in the planning and organization of programs designed to maximize patient support and care delivery.

• Engage in regular communication with physicians to report on patient progress, address needs, discuss any concerns, and identify potential challenges.

• Maintain accurate, timely, and confidential records including clinical assessments and progress notes, adhering to departmental policies and regulations.
\n
• Collaborate closely with other health care team members to develop comprehensive treatment plans based on detailed psychosocial assessments, including discharge planning.

• Provide targeted counseling and crisis intervention to patients, focusing on the evaluation and management of emotional and behavioral challenges.

• Consult with and coordinate care involving family members and support systems, ensuring a holistic approach to treatment and planning for discharge.

Patient Population / Client Focus\n

You will primarily serve high-risk patients within a hospice center environment, encompassing those facing complex emotional and psychosocial challenges related to serious illness. Your role will be instrumental in helping patients and their families navigate difficult situations, providing them with resources and support aimed at enhancing their quality of life during sensitive times.

Qualifications\n

• A minimum of 3 years of experience in an outpatient clinical setting, preferably with a focus on mental health.

• A Master’s degree or Doctorate in Social Work or a related field is required; candidates with advanced degrees will be prioritized.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
\n
• Strong ability to work both independently and as part of a collaborative team.

• Fluency in written and spoken English, with exceptional communication skills.

• Proficient in computer applications, including MS Office and electronic health record (EHR) systems.

Preferred Experience\n

Compensation and Benefits\n
