This full-time, remote position involves working within a community health clinic setting to provide essential support and guidance to seniors and their families. Work Environment\n

This position will be primarily remote, allowing flexibility while still providing essential services at the community health clinic. The work schedule typically spans Monday to Friday from 8:00 AM to 4:30 PM, although occasional evening or weekend hours may be required based on client needs. This flexibility is critical to meet the demands of our clients and ensure continuity of care.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate seniors' emotional, mental, and social needs.

• Develop and implement personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.
\n
• Provide counseling and emotional support, assisting clients through life transitions, such as health changes, bereavement, or isolation.
\n
• Advocate for clients by ensuring access to community resources, services, and programs.
\n
• Collaborate with an interdisciplinary team, including nurses and healthcare professionals, to improve client care and outcomes.
\n
• Regularly visit clients to monitor their well-being, assess living conditions, and address any safety issues.
\n
• Educate clients and their families on available community support services to foster social engagement.
\n
• Maintain accurate documentation of client interactions, assessments, and progress in compliance with privacy regulations.

• Stay current on social work practices and ethical standards to better serve clients.

Patient Population / Client Focus\n

The primary focus of this role is on seniors within the community who may be facing various challenges such as mental health issues, social isolation, or the complexities of aging. The Social Worker will work closely with seniors and their families to navigate these challenges, ensuring they have access to necessary resources and support systems.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• Minimum of two years of experience in a social work role, ideally within a community health clinic or geriatric setting.

• Strong knowledge of social work principles, aging-related issues, and community resources.
\n
• Excellent assessment, counseling, and conflict resolution skills.

Compensation and Benefits\n
