Key Responsibilities\n

• Conduct thorough assessments of clients’ social, emotional, and environmental challenges, determining the most effective strategies to support their needs.

• Provide direct counseling and emotional support to individuals and families navigating challenges related to health, housing, and psychological well-being.

• Engage with clients in crafting personalized service plans that articulate their needs, aspirations, and desired outcomes, ensuring their voices are at the forefront of the planning process.

• Implement crisis interventions and conduct risk assessments, including safety planning when necessary, to maintain client safety and stability.

• Collaborate with various community agencies to create a seamless support network for clients and advocate for their rights and access to resources.

• Participate actively in community outreach initiatives, enhancing public awareness of the services offered by Health Access Network and fostering trust with the community.

The LCSW plays a key role in addressing the needs of diverse clients, including individuals facing chronic illness, those impacted by grief and loss, and families dealing with multifaceted social determinants of health. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in New York State.

Compensation and Benefits\n
