Key Responsibilities\n

• Conduct individual and group therapy sessions employing evidence-based therapeutic modalities such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Facilitate family counseling sessions aimed at resolving conflicts and enhancing family dynamics.

• Collaborate closely with an integrated team of healthcare professionals—psychiatrists, nurses, and primary care physicians—to develop personalized treatment plans that foster optimal health outcomes.

• Provide timely crisis intervention services, ensuring clients receive appropriate referrals for further assistance when needed.

• Maintain meticulous documentation of patient records as per HIPAA regulations, ensuring all information is accurate, secure, and accessible.

• Actively participate in staff meetings, case conferences, and peer supervision sessions to promote continuous learning and improvement within our practice.

Patient Population / Client Focus\n

This role will involve working primarily with adult clients facing a range of mental health issues, including but not limited to anxiety, depression, chronic illness, and grief associated with end-of-life care. Understanding the unique challenges faced by individuals in hospice settings is crucial, as the Social Worker will be integral in providing emotional, psychological, and practical support to both patients and their families during critical times.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from $80,000 to $100,000, commensurate with experience and qualifications.
