Key Responsibilities\n

• Conduct comprehensive assessments to identify patients' needs and develop individualized treatment plans.

• Maintain clear communication with supervisors, direct care staff, as well as family members to discuss client progress and care strategies.

• Prepare and manage a tentative schedule of client appointments, home visits, and observations to ensure thorough monitoring of progress.
\n
• Coordinate home visits, conduct in-home observations, perform chart reviews, and engage in data analysis to support treatment objectives.
\n
• Ensure that all program plans are implemented effectively and efficiently in accordance with best practices.
\n
• Participate actively in interdisciplinary team meetings, contributing insights, and monitoring patient outcomes.
\n
• Complete monthly and quarterly progress notes that detail each client’s therapeutic journey and progress.
\n
• Organize training sessions to ensure that staff and colleagues are well-versed in the implementation of treatment plans and documentation protocols.
\n
• Conduct follow-up interviews and offer continued support through home visits, ensuring that clients’ evolving needs are met.
\n
• Maintain thorough and organized records for ease of access by relevant stakeholders.
\n
• Act as a liaison with the public and mental health community to enhance collaborative efforts in expanding service capabilities.

• Assist in developing training materials aimed at improving program delivery and overall client outcomes.

Patient Population / Client Focus\n

As an LPC at Primary Counseling Center, you will work with a diverse clientele, including adults and youth facing various mental health challenges. Our focus includes, but is not limited to, issues such as anxiety, depression, trauma, substance misuse, and relational dynamics. Your expertise in these areas will contribute to crafting impactful therapeutic experiences tailored to each individual’s unique circumstances.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Must hold an active and unrestricted Professional Counselor (LPC) license in the state of Arizona.

• At least two years of relevant counseling experience, particularly in a clinical environment, is essential.

• A strong familiarity with legal and ethical standards in mental health practice is required.

• Proficient interpersonal and communication skills are necessary to foster rapport with a wide range of clients.

• Proven ability to work collaboratively within a healthcare team setting.

• Experience with Electronic Medical Record (EMR) software and related technologies is highly preferred.
