Work Environment

This position is onsite, focusing primarily on home care, allowing you to provide personalized treatment in the most comfortable settings for your patients. Key Responsibilities

• Conduct comprehensive evaluations of patients to establish baseline functional status and identify needs for treatment.
• Develop and implement individualized treatment plans that address patients' specific functional limitations and disabilities.
• Monitor and document patient progress, adjusting treatment plans as necessary and ensuring all documentation complies with professional standards, laws, and regulations.
• Provide education and consultation to healthcare providers regarding patients' treatment protocols and continuity of care.
• Actively collaborate with interdisciplinary team members, including physicians and other specialists, to enhance patient care and outcomes.
• Engage in community outreach and participate in leadership roles within the organization, contributing to quality improvement initiatives and program development.
• Ensure effective verbal and written communication with patients, families, and support staff regarding treatment plans and patient progress.
• Supervise and mentor support staff, fostering a collaborative and educational work environment.
• Participate in ongoing education through self-study and formal training opportunities to maintain professional competence and licensure.
• Maintain strong relationships with patients and their families, involving them in treatment decisions and discharge planning.

Patient Population / Client Focus

Our physical therapists work primarily with patients recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. This role necessitates a patient-centered approach, adapting treatments to cater to varied needs and ages. You will empower patients through education and personalized care, making significant contributions to their emotional and physical recovery.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• An active State License in Physical Therapy demonstrating your qualification to practice is essential.
• BLS certification (Basic Life Support) is mandatory to ensure patient safety and preparedness.
• Strong written and verbal communication skills are crucial for effective patient interaction and collaboration with the healthcare team.
• Proficiency in word processing and familiarity with electronic medical record systems are necessary for accurate documentation and reporting.

Preferred Experience

• Experience in home health or outpatient orthopedic settings is preferred but not mandatory.
• Prior involvement in community health outreach programs is a plus.
• Demonstrable leadership experience in mentoring staff or in multidisciplinary team settings will set you apart.

Compensation and Benefits
