Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments to evaluate mental health, substance abuse, and medical needs of patients within a primary care framework.

• Document mental health evaluations, including progress notes and counseling sessions, using electronic health record (EHR) systems to ensure accurate tracking of patient care.

• Provide therapeutic services to individuals, couples, and families, encompassing intake assessments, diagnosis, treatment planning, and crisis intervention.
\n
• Facilitate support groups and educational sessions to enhance patient coping mechanisms and resilience.
\n
• Act as a liaison between patients and community resources, assisting with referrals for services such as medication assistance programs and additional mental health support.

• Collaborate closely with Quality Improvement teams to contribute to data collection efforts that enhance service delivery and patient outcomes.

Your primary focus will be on patients and families navigating the complexities of hospice care. You will work with diverse individuals who are dealing with serious illnesses, offering empathetic support and guidance as they confront end-of-life challenges. Qualifications\n

• A Master’s degree in Social Work and current designation as a Licensed Clinical Social Worker (LCSW) are mandatory.

• Three years of experience in social work post-licensure, ideally within mental health or clinical settings.

• Strong familiarity with counseling techniques and crisis intervention methods, along with the ability to make sound independent judgments related to patient care.

• Excellent communication skills to effectively articulate patient needs and coordinate with a multidisciplinary team.

• Proficient in using EHR systems for documentation and tracking patient progress.

Preferred Experience\n

• At least two years of experience specifically in mental health or clinical settings, with a focus on group facilitation and planning.

• Experience working with hospice or palliative care populations is highly desirable.

Compensation and Benefits\n
