Key Responsibilities\n

• Conduct detailed psychosocial assessments of clients to understand their emotional and mental health concerns, strengths, and available resources.

• Provide therapeutic interventions, including individual, family, and group therapy, utilizing evidence-based approaches such as cognitive-behavioral therapy and trauma-focused therapy.

• Design and implement personalized treatment plans that align with clients’ goals and interventions tailored specifically for their situations.

• Work collaboratively with a multidisciplinary treatment team consisting of psychiatrists, psychologists, and other professionals to deliver coordinated care.

• Facilitate therapeutic group sessions, psychoeducation classes, and support groups while creating a safe and inclusive environment for all participants.

• Respond to clients' immediate needs through crisis intervention, risk assessments, and safety planning when required.
\n
• Maintain thorough and timely documentation in compliance with ethical, legal, and professional standards, ensuring accurate records of each client’s progress and treatment.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals requiring mental health support within a hospice setting. This may include clients coping with terminal illness, their families dealing with grief and loss, and addressing a variety of mental health conditions. You will employ a compassionate, client-centered approach, ensuring that all interactions reflect dignity and respect.

Qualifications\n

• Master’s degree in Social Work from an accredited program.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.

• A minimum of two years of relevant experience in a mental health or outpatient care setting, preferably within hospice or palliative care.

• Demonstrated knowledge of evidence-based therapeutic modalities and treatment interventions.

Preferred Experience\n

Compensation and Benefits\n
