Key Responsibilities\n

• Work alongside medical teams to develop and implement effective discharge plans that ensure continuity of care post-hospitalization.

• Assist patients and their families in navigating treatment options and connecting with community resources for ongoing support.

• Deliver crisis intervention and emotional support to patients facing high-stress situations.

• Document patient interactions and ensure compliance with all relevant hospital policies and legal standards.
