Key Responsibilities\n

As an LCSW at Sondermind, you will play a vital role in delivering therapeutic services to clients undergoing hospice care. Your responsibilities will include:

• Conducting comprehensive assessments and developing tailored treatment plans for individual and couple therapy sessions.

• Facilitating therapy sessions, utilizing evidence-based interventions to help clients navigate through their emotional and psychological challenges.

• Maintaining meticulous records of all therapy sessions, including progress notes and treatment adjustments, ensuring timely documentation to comply with regulatory standards.

• Managing client scheduling and maintaining an organized calendar to ensure all ongoing appointments are met transparently and efficiently.

• Collaborating with multidisciplinary teams, including medical professionals and hospice staff, to deliver integrated care that addresses both emotional and physical health needs.

Patient Population / Client Focus\n

Your focus will be on clients within the hospice setting, including individuals from diverse backgrounds and age groups confronting terminal illnesses. Qualifications\n

To be successful in this role, candidates must possess:

• A Bachelor's degree in Social Work, Psychology, or a related field.

• A valid LCSW license to practice independently in California.

Compensation and Benefits\n
