Key Responsibilities Conduct thorough clinical evaluations of patients to assess their emotional and psychological needs, developing tailored therapeutic interventions. Facilitate psychoeducational and process-oriented therapy groups for both patients and families, addressing coping strategies and emotional challenges related to end-of-life issues. Complete discharge planning sessions with patients and their families to ensure a clear understanding of coping skills, transition plans, and available resources. Confirm discharge appointments with patients and their loved ones, enhancing communication and mental preparation for the transition. Network effectively with referral sources and outpatient providers to ensure continuity of care during admission, at midpoint assessments, and upon discharge. Provide case management support by facilitating patient transfers to higher levels of care when necessary. Actively participate in clinical outreach initiatives and program tours, enhancing the community's awareness of hospice services. Patient Population / Client Focus The focus of this role is primarily on patients receiving hospice care, which involves working with individuals facing terminal illnesses and their families. This setting requires a unique approach, as it encompasses emotional support, guidance through grief, and assistance in navigating the complexities of end-of-life decisions. Qualifications A Master’s degree in Social Work (MSW) from an accredited institution. Current active licensure as a Clinical Social Worker (LCSW) in California. Exceptional written and verbal communication skills, enabling effective interaction with patients, families, and interdisciplinary teams. Ability to maintain professional boundaries and model appropriate behavior within therapeutic relationships. Strong organizational skills to ensure timely documentation and submission of service-related paperwork. Preferred Experience Previous experience in hospice or palliative care settings, demonstrating a strong understanding of end-of-life issues and terminal patient needs. Familiarity with telehealth technologies and electronic health record systems to enhance service delivery. Experience in leading group therapy sessions and conducting psychoeducational workshops.
