Key Responsibilities\n

As an LPC at Motion Matrix Counseling, you will:

• Conduct comprehensive assessments and evaluations to identify patients' needs and develop tailored treatment plans.

• Maintain ongoing communication with supervisors, direct care team members, and family members to discuss client progress and address any emerging concerns.
\n
• Develop and provide a structured schedule for appointments, home visits, and observation sessions to systematically monitor client progress.
\n
• Coordinate and execute home visits, facilitating in-home assessments and collaborative sessions with family and support systems.
\n
• Ensure the effective implementation of treatment plans and program protocols, adjusting them as necessary based on patient feedback and progress.
\n
• Engage actively in Interdisciplinary Team Meetings to discuss individual cases and share insights on program effectiveness.
\n
• Document monthly and quarterly progress, ensuring that records are thorough and accessible for all team members involved in patient care.
\n
• Organize training sessions for staff to ensure competence in program implementation and the accurate completion of documentation.
\n
• Provide ongoing support including follow-up interviews and home visits to maintain progress and adjust treatment as needed.

• Act as a liaison to the community and other mental health organizations, fostering relationships that support development initiatives.

• Assist in refining training materials and procedures to enhance program effectiveness for individuals served.

Patient Population / Client Focus\n

Qualifications\n

To be successful in this role, candidates must have:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.

• A minimum of two years of experience in counseling, preferably in a clinical setting.
