Key Responsibilities

• Direct physical therapy treatment plans to prevent disability, alleviate pain, and restore muscle function tailored to the unique needs of each client.
• Oversee the implementation of treatment plans by physical therapy assistants, ensuring adherence to prescribed protocols.
• Conduct comprehensive evaluations of client capabilities through diagnostic tests assessing muscle, nerve, and joint functions.
• Develop recommendations to enhance the client's independence based on evaluations of their home environment.
• Deliver in-home physical therapy sessions as directed by physician referrals, ensuring high-quality care.
• Administer manual therapeutic exercises, as well as instruct clients in non-manual exercises and daily functional activities to promote recovery.
• Use modalities such as traction and massage during therapy sessions, applying appropriate physical agents as needed.
• Evaluate, fit, and make necessary adjustments to prosthetics and orthotic devices while collaborating closely with physicians.
• Regularly update client care plans through participation in interdisciplinary team meetings and case conferences.
• Thoroughly document client progress, including initial evaluations, progress notes, and discharge summaries, and submit them to the clinical record.
• Engage with client families to provide education and support regarding therapy devices and the client's therapeutic program.
• Remain readily available to assist Physical Therapy Assistants during client treatments, ensuring compliance with state laws.
• Remain adaptable, taking on additional tasks as assigned, to contribute to the company's overall objectives.

Patient Population / Client Focus

In this role, you will work with a diverse range of patients, from elderly clients with mobility challenges to post-operative patients needing rehabilitation. Our focus is on creating a supportive environment tailored to the individual needs of each client. We emphasize personalized care strategies, allowing our therapists to fully engage with clients and families to achieve optimal recovery outcomes.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Active, state-issued Physical Therapist licensure in good standing.
• One year of experience in physical therapy, ideally in an outpatient or rehabilitation setting.
• Current CPR certification to ensure safety in emergency situations.
• Strong commitment to clinical excellence, quality care, and the enhancement of the client experience.
• Excellent interpersonal skills, demonstrating empathy and the ability to foster meaningful relationships with clients and their families.
• Proficiency in maintaining accurate clinical documentation and adhering to relevant regulatory standards.
• Proficient in utilizing equipment and therapy modalities relevant to the field of physical therapy.

Preferred Experience

• Experience in an acute care or home care setting will be highly regarded.
• Familiarity with electronic health record (EHR) systems to streamline documentation and communication.
