Key Responsibilities\n

• Conduct thorough evaluations and assessments of young clients to evaluate their developmental needs.
\n
• Participate in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring alignment with each child and family's unique priorities.
\n
• Regularly monitor outcomes and provide feedback to team members regarding the effectiveness of developmental strategies.
\n
• Engage with families, offering guidance, education, and support designed to enhance the child's growth and development in natural settings.
\n
• Follow federal and state regulations to maintain compliance while delivering high-quality care.
\n
• Utilize a coaching model to facilitate family involvement in the child's development process.
\n
• Develop rapport with children and families, creating a trusting and supportive therapeutic relationship.

Patient Population / Client Focus\n

Your primary focus will be on children—particularly those under the age of three—who are experiencing delays in physical, cognitive, social/emotional, and communication skills. By working in a community health clinic setting, you will have the opportunity to provide direct support to families, emphasizing early intervention strategies that promote healthy developmental outcomes.

Qualifications\n

• A Master’s Degree or Doctorate of Social Work from an accredited institution is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

• A valid driver’s license for potential travel to client homes, though primarily remote work is expected.
