Key Responsibilities\n

• Conduct thorough assessments of clients' counseling needs, establishing individualized treatment plans that address their specific challenges.

• Utilize DSM (Diagnostic and Statistical Manual) criteria to evaluate clients for substance abuse issues, guiding decisions regarding the appropriate level of care in alignment with ASAM (American Society of Addiction Medicine) standards.

• Prepare and implement the substance abuse counseling segment of the client treatment plan, ensuring that interventions are timely and relevant.
\n
• Develop discharge and continuum of care plans that facilitate smooth transitions for clients into the appropriate levels of support as necessary.
\n
• Meticulously document all interactions, including counseling sessions, educational services, assessments, and follow-ups, in clients' clinical records to maintain an accurate account of their treatment journey.
\n
• Maintain an active client caseload, efficiently managing case reviews, treatment planning, and clinical charting as required.
\n
• Coordinate with family and other support services, meticulously recording services rendered to provide a cohesive treatment experience for clients.
\n
• Engage with external referral sources such as schools, legal entities, and employers to ensure comprehensive support for clients.

• Facilitate interventions during crisis situations, employing a multi-disciplinary team approach to address urgent client needs effectively.

Patient Population / Client Focus\n

The LCSW will work predominantly with students in the school setting, many of whom may be facing co-occurring mental health and substance use issues. This role requires an understanding of the unique challenges faced by youth, including the pressures of academic performance, social dynamics, and potential familial issues.
