Work Environment

This role will involve working primarily onsite at patients' homes across North Greece, NY. Our physical therapists have the autonomy to manage their schedule while ensuring comprehensive assessments and treatments align with the patients' needs and home environment. You will collaborate closely with an interdisciplinary team, including occupational therapists and nursing staff, fostering a supportive and cooperative work atmosphere where patient-centric care is the priority.

Key Responsibilities

• Conduct thorough evaluations of residents to assess physical abilities, limitations, and treatment requirements.
• Develop and implement customized treatment plans focusing on improving strength, mobility, and overall functional independence through specific interventions and therapeutic exercises.
• Utilize evidence-based techniques, including manual therapy and therapeutic modalities, to effectively address physical impairments.
• Monitor patient progress through diligent documentation and adjust treatment strategies as needed to optimize outcomes.
• Educate residents and their families on home exercise programs, safety measures, and strategies to maintain long-term physical health.
• Foster communication and collaboration with an interdisciplinary care team, sharing insights and strategies to deliver comprehensive patient care.
• Ensure adherence to all regulatory standards and professional practice guidelines to maintain the highest level of safety and quality of care.

Patient Population / Client Focus

In this role, you will primarily work with a diverse range of patients, including older adults with mobility challenges, post-operative individuals, and those recovering from injuries. The home care setting presents unique challenges and opportunities to provide therapeutic interventions tailored to each patient's lifestyle, ensuring that rehabilitative care is both effective and practical.

Qualifications

• A current and valid Physical Therapist license in New York State.
• A Bachelor's Degree in Physical Therapy from an accredited program; Master's or Doctorate in Physical Therapy is preferred.
• Strong communication, interpersonal, and organizational skills, necessary for effective patient interaction and teamwork.
• A dedication to continuous professional development and a proactive approach to staying current in the field of physical therapy.

Preferred Experience

• A minimum of two years of clinical experience as a Physical Therapist, ideally with experience in home care or skilled nursing settings.
• Experience working with the geriatric population, addressing age-related physical issues, and adapting therapeutic exercises to their needs.

Compensation and Benefits

Strengthen Physical Therapy, Inc. offers a competitive annual salary ranging from USD 90,000 to 110,000, commensurate with experience and qualifications.
