Work Environment

This full-time role is based onsite within our Home Care setting, allowing for direct patient interaction in a supportive and health-focused environment. Our facility spans over 5000 square feet dedicated to sports medicine, equipped with cutting-edge therapeutic and rehabilitation tools. You will be part of a dynamic team comprised of fellow physical therapists and skilled support staff, all committed to fostering patient well-being.

Key Responsibilities

• Conduct thorough patient assessments to identify rehabilitation objectives and health requirements in a home care setting.
• Effectively manage a comprehensive patient load, focusing on one patient per hour to deliver quality care.
• Design and implement patient-specific care plans utilizing evidence-based treatment protocols.
• Utilize a variety of techniques, including manual therapy, to enhance patients' mobility while minimizing reliance on medicative or surgical interventions.
• Motivate and educate patients throughout their rehabilitation process to maximize their physical capabilities and independence.
• Encourage a healthy lifestyle by developing tailored fitness and wellness programs addressing each patient's unique needs.
• Collaborate closely with interdisciplinary healthcare professionals to ensure holistic and coordinated patient care.
• Regularly evaluate treatment outcomes, monitor patients' progress, and adjust plans as necessary.
• Maintain accurate and comprehensive treatment documentation in line with best practices and legal requirements.

Patient Population / Client Focus

Our primary focus is on patients with orthopedic conditions seeking outpatient physical therapy. The patient demographic largely consists of individuals with private health insurance, emphasizing tailored treatment approaches to each person's situation. Your expert guidance will significantly contribute to transforming their physical health and overall quality of life.

Qualifications

• Possession of a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Valid certification as a licensed Physical Therapist in the state of New York.
• Clean professional record with no disciplinary actions against your license.
• Strong manual therapy skills with extensive knowledge of intervention techniques.
• Proficiency in assessing patients and developing appropriate exercise regimens tailored to individual needs.
• Experience with Electronic Medical Records (EMR) systems, particularly platforms like WebPT.

Preferred Experience

• Previous experience working in orthopedic outpatient settings is highly desirable.
• Demonstrated ability to implement diverse treatment techniques effectively.

Compensation and Benefits
