This position operates primarily in a home care setting, where you will work directly within the comfort of patients' homes across Port Charlotte and surrounding areas. Key Responsibilities

• Conduct thorough examinations and evaluations of patients with functional limitations and disabilities to create personalized treatment plans that promote recovery and mobility.

• Adhere strictly to state practice acts, ensuring compliance with laws that govern the scope of physical therapy practice and the supervision of aides or assistants.

• Utilize evidence-based practice to implement individualized treatment protocols, modifying approaches as necessary based on patient progress and evolving healthcare standards.

• Collaborate with a multidisciplinary team, recognizing when to consult with other healthcare professionals regarding changes in a patient's condition that may require additional medical or rehabilitative intervention.

• Maintain comprehensive documentation of all patient interactions, evaluations, treatment plans, and progress, ensuring accuracy and clarity to support the patient's case and treatment course.

• Engage in effective discharge planning and follow-up care, ensuring patients understand their treatment outcomes and necessary next steps for continued recovery.

• Stay informed of the latest research and trends in physical therapy, participating in in-services, training events, and team meetings to enhance team collaboration and patient care practices.

• Deliver all therapies assigned daily while adhering to quality standards and maximizing departmental productivity.

• Support initiatives that remove barriers to patient access, leveraging skilled knowledge of access metrics to foster inclusivity in healthcare.

Patient Population / Client Focus

The patients you will serve range widely in age and condition, often dealing with chronic pain, post-surgical recovery, or age-related mobility issues. Your role will involve not just skillful assessment and treatment, but also building strong relationships to empower patients as they navigate their recovery processes. Providing them with empathy and understanding will enhance their overall experience and outcomes.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• Current valid state license to practice as a Physical Therapist in Florida.

• Minimum of two years of experience in a clinical setting, particularly within home care or a similar environment.

• Current certification in CPR.

• Strong communication skills, and the ability to form rapport quickly with patients and their families.

Preferred Experience

• Experience with digital health solutions and related software systems that improve patient engagement and care delivery.

• Previous experience in a home care setting, demonstrating the ability to work independently in various environmental contexts.

Compensation and Benefits
