Work Environment

This position is based in the Punta Gorda area, where you will deliver patient care directly in their homes. Our home care environment allows you to create personalized therapeutic experiences, addressing the unique challenges and needs of each patient. You will work from Monday to Friday, with a structured schedule from 9 AM to 5 PM, providing work-life balance while ensuring high-quality patient interactions.

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities and challenges to create customized treatment plans.
• Provide individualized therapy sessions, focusing on a range of orthopedic and neurological disorders.
• Maintain a manageable caseload, ensuring that each patient receives the requisite attention and personalized care they deserve.
• Educate patients and their families on therapeutic exercises and lifestyle modifications that promote healing and functional independence.
• Implement evidence-based interventions that are designed to reduce pain, enhance mobility, and improve overall function.
• Facilitate non-surgical rehabilitation through innovative therapies and strategic exercises tailored to individual recovery goals.
• Monitor and document patient progress meticulously, ensuring up-to-date records in accordance with healthcare regulations and standards.
• Collaborate effectively with other healthcare professionals, including physicians, nurses, and rehabilitation specialists, to provide cohesive care.
• Encourage and motivate patients throughout their treatment journey, adapting intervention strategies as needed for optimal outcomes.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, ranging from post-surgical rehabilitation to those with chronic conditions affecting mobility. Your expertise and empathy will be crucial in helping patients regain their independence and enhance their quality of life through tailored therapeutic interventions.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active Physical Therapist license in the state of Florida, with no history of sanctions or disciplinary actions.
• Proven proficiency in various manual therapy techniques and modalities.
• Strong background in developing and implementing successful exercise programs suited to individual patient needs.
• Competence in using electronic medical records (EMR) software, particularly Web PT, for accurate patient documentation.
• Commitment to staying informed on advancements in physical therapy techniques and methodologies.

Preferred Experience

• Prior experience in home health care settings is highly desirable.
• Experience working with a broad range of patient diagnoses, particularly those requiring physical rehabilitation.
• Demonstrated success in patient engagement and motivation through innovative therapeutic strategies.

Compensation and Benefits

We offer a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
