Work Environment

This position is based in our Port Charlotte rehabilitation facility, which is designed to provide a welcoming and effective space for both patients and staff. You will work onsite Monday through Friday, from 8 AM to 5 PM, collaborating with a multidisciplinary team of healthcare professionals to ensure optimal patient care. Our facility is equipped with the latest technologies and treatment methodologies to support our therapists in delivering high-quality therapy.

Key Responsibilities

• Conduct comprehensive patient evaluations to assess individual needs and establish tailored treatment plans that align with patients' goals and conditions.

• Implement treatment protocols based on evaluation findings while adapting to any changes in patients' conditions.

• Collaborate with other healthcare providers and specialists to optimize care and refer patients for additional medical consultations when necessary.

• Document patient progress through detailed daily notes, interim progress summaries, and discharge evaluations, ensuring compliance with regulatory standards.

• Play an active role in discharge planning, ensuring that patients have clear follow-up instructions and are set up for continued success post-therapy.

• Maintain open lines of communication with patients, family members, and third-party payors to enhance patient satisfaction and understanding of care processes.

• Participate in facility meetings and contribute to ongoing training and development efforts to ensure that clinical practices meet or exceed state and federal regulations.

Patient Population / Client Focus

You will primarily engage with orthopedic patients who require physical therapy for various musculoskeletal injuries and conditions. This may include post-operative rehabilitation, sports injuries, chronic pain management, and functional limitations affecting daily living activities. Your role will significantly contribute to the recovery and improved quality of life for each patient.

Qualifications

• Doctorate Degree in Physical Therapy (DPT) from an accredited institution.

• Valid physical therapy license in the state of Florida or eligibility to obtain one.

• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is required.

Preferred Experience

• At least 2 years of experience in an outpatient orthopedic setting is highly desirable.

• Experience with electronic medical records and patient management software is a plus.

Compensation and Benefits
