Work Environment

This position is centered around a collaborative and supportive work environment, primarily conducted onsite in patients' homes across Punta Gorda, Florida. Key Responsibilities

• Design and implement individualized physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and restoring functional abilities as per physician's prescriptions.
• Conduct comprehensive assessments of clients, utilizing diagnostic tests to evaluate muscle, nerve, joint function, and overall physical capabilities.
• Oversee and mentor physical therapy assistants, ensuring they follow treatment protocols and effectively support patient care.
• Analyze clients' home environments and recommend adaptations and strategies to enhance their independence during rehabilitation.
• Utilize manual therapy techniques, therapeutic exercises, and modalities such as traction and massage while educating clients and families on their roles in the therapy process.
• Maintain detailed clinical records, including initial evaluations, treatment plans, progress notes, and discharge summaries, and submit them in a timely manner.
• Engage in regular interdisciplinary case conferences, collaborating with healthcare team members to optimize treatment plans and address any changes in the patient's condition.
• Administer and adjust prosthetic and orthotic devices in conjunction with physician recommendations.
• Provide education and training to clients and their families on the use and maintenance of therapy equipment.
• Complete functional assessments and ensure optimal utilization of community resources to support patients' rehabilitation goals.

Patient Population / Client Focus

You will primarily work with a diverse range of patients, including those recovering from surgeries, managing chronic health conditions, and supporting geriatric clients. Our goal is to provide compassionate, tailored therapy that honors each client's individual journey towards recovery and independence.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• A minimum of one year of experience in a physical therapy setting, preferably in home health or acute care.
• Current and valid Physical Therapist licensure in Florida.
• Current CPR certification and a valid driver's license with automobile insurance.
• Exemplary communication and interpersonal skills, with a strong emphasis on patient and family education.
• Ability to make informed clinical decisions and adapt treatment plans according to patient progress.

Preferred Experience

• Experience with home care physical therapy practices and protocols.
• Familiarity with electronic medical record (EMR) systems to efficiently document and track patient care.
• Prior involvement in interdisciplinary team collaboration and care planning.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
