Dates of Coverage: ASAP until March 2026! Approx. 7-10+ days per month, ongoing!

Schedule:

• Hospital: M-F
• Clinic: Tues & Thurs + possible night call

Practice Type: Office/Clinic & Hospital

Caseload:

• Practice Percentage: Adult 100%; Trauma 10%; Office/Clinic 25%; Hospital 75%
• Average No. of office/clinic patients per physician per day: 10; Scheduled - 10; Add-ons - 1
• Average No. of hospital patients per physician day: 5-10; Admissions: 1-2; ICU: 2; Surgeries: 1-5

Worksite Resources:

• 4 Exam Rooms
• Lab & X-ray onsite: No
• Office ancillary staff: 1 LPN, 1 scheduler, 1 APP in clinic
• Documentation: EPIC (Office/Clinic & Hospital)

Cardiac/Thoracic Required Procedures:

• General CV/thoracic surgery* open
• Coronary artery bypass surgery on pump
• Valvular repair & replacement

Cardiac/Thoracic Preferred Procedures:

• Congenital defects (ASD, VSD, PFO, etc.)
• Aneurysm repair cardiac & aorto-thoracic

Submission Requirements:

• Board Certified (Thoracic) - REQUIRED
• CO License - REQUIRED
• BLS, ACLS - REQUIRED
• Flu Vaccine - REQUIRED
• Must be able to provide 7-10+ days of coverage per month - REQUIRED (Note at time of name clear)
