Key Responsibilities\n

As a Licensed Professional Clinical Counselor, your primary responsibilities will include:

• Assisting primary care providers in identifying and managing mental health and psychosocial issues, contributing your expertise to the primary care team.

• Conducting patient intakes that involve comprehensive diagnostic and functional evaluations, followed by making informed recommendations for treatment plans.

• Providing brief interventions for patients in need of mental health services, ensuring they receive timely support.

• Guiding the primary care provider in deciding which patients would benefit from continued care within the clinic and which should be referred to specialized mental health facilities.

• Implementing follow-ups to monitor patient progress, assessing the effectiveness of treatments and adjusting care plans as necessary.

• Developing and implementing relapse prevention plans for patients to maintain stable functioning in their daily lives.

• Identifying at-risk patients, creating preventative strategies to avert the deterioration of their health.

• Coordinating behavioral health services among healthcare providers, ensuring a seamless connection for patients, including those outside our practice with their consent.

• Maintaining detailed documentation of patient progress, treatment plans, and diagnostic information.
\n
• Collaborating with primary care providers to foster effective relationships and solutions that meet each patient’s unique mental health needs.

• Equipping patients with self-management skills, educational support, and coping strategies for any medical treatments initiated by the primary care team.

• Providing consultation to clinic management and team members, contributing insights on behavioral services and program evaluations.

• Complying with the Code of Ethical Conduct for Clinical Professionals and performing additional duties as assigned.
