St. Elara Medical Center is seeking a dedicated and compassionate Physical Therapist to join our team in providing high-quality therapy services within assisted living facilities in Ridgewood, New Jersey. Work Environment

This position is primarily onsite within various assisted living facilities across the region, allowing therapists to develop meaningful relationships with residents in a home-like environment. Key Responsibilities

• Provide direct, on-site physical therapy services to residents in our affiliated assisted living communities, creating a comfortable atmosphere for rehabilitation.
• Conduct thorough evaluations to assess each resident's functional abilities, limitations, and orthopedic conditions, leading to the development of individualized treatment plans tailored to their unique needs.
• Collaborate effectively with nursing staff, caregivers, and the wider team of medical professionals to coordinate care and track residents' progress, ensuring seamless communication and continuity of care.
• Educate residents and their families about safe mobility practices, home exercises, and wellness strategies to enhance their quality of life, empowering them to take an active role in their rehabilitation.
• Utilize mobile electronic medical records (EMR) for efficient and secure documentation of patient interactions and treatment plans, ensuring compliance with HIPAA regulations.
• Act as a liaison between the assisted living facilities and our multidisciplinary clinic regarding referrals and follow-up appointments, promoting a cohesive care experience.

Patient Population / Client Focus

Our focus is predominantly on older adults residing in assisted living facilities, many of whom may struggle with mobility and require rehabilitation related to orthopedic conditions. Understanding the unique challenges faced by the elderly is crucial, as each individual presents their own set of needs that require tailored interventions. Your role will be vital in enhancing their independence and overall well-being through targeted therapeutic interventions.

Qualifications

• Valid Physical Therapist license in the State of New Jersey or eligibility to obtain licensure.
• A Bachelor's degree in Physical Therapy from an accredited program.
• Strong interpersonal and communication skills, fostering a supportive environment for residents and their families.

Preferred Experience

• A minimum of two years of experience in outpatient settings, preferably with a focus on geriatrics, assisted living facilities (ALF), skilled nursing facilities (SNF), or home care.
• In-depth clinical knowledge of common orthopedic conditions affecting older adults to ensure tailored, effective interventions.
• Comfort and experience working autonomously within a residential environment, demonstrating adaptability and initiative.

Compensation and Benefits

• Competitive salary range of $90,000 to $100,000 per year, commensurate with experience and qualifications.
