Work Environment

This position is primarily onsite, working within various assisted living facilities across Bogota and the surrounding areas. Key Responsibilities

• Deliver on-site therapy services to residents in assisted living facilities, focusing on individualized care tailored to each resident's unique needs.
• Conduct thorough evaluations of residents' physical capabilities and limitations to develop orthopedic-informed treatment plans that enhance mobility and overall well-being.
• Collaborate seamlessly with facility nursing staff, families, caregivers, and our internal team of medical professionals to ensure comprehensive and coordinated care.
• Educate residents and their families about safe mobility practices, home exercise programs, and wellness strategies to encourage independence and enhance quality of life.
• Utilize mobile EMR tools for efficient documentation, ensuring compliance with HIPAA while streamlining patient records and progress tracking.
• Act as a liaison between the facility and our multidisciplinary clinic, facilitating referrals, follow-ups, and continuity of care.

Patient Population / Client Focus

The primary focus of your practice will be on the geriatric population residing in assisted living facilities. This demographic often presents with orthopedic conditions related to aging, such as arthritis, post-surgical rehabilitation needs, and mobility challenges. Your role will involve not just physical rehabilitation, but also fostering connections that promote emotional support and enhance residents' ongoing engagement in life.

Qualifications

• A valid Physical Therapist license in the State of New Jersey, or eligibility to obtain licensure.
• A Bachelor's Degree in Physical Therapy or a related field.
• Demonstrated exceptional interpersonal and communication skills to effectively engage with residents, families, and caregivers.

Preferred Experience

• At least 2 years of experience in outpatient, geriatric care, or assisted living settings preferred.
• A strong clinical understanding of orthopedic conditions prevalent among aging adults, enhancing treatment efficacy and patient outcomes.
• Previous experience working autonomously in residential environments will be advantageous.

Compensation and Benefits

The compensation for this position is competitive, with a salary ranging from $90,000 to $100,000 per year, commensurate with experience.
