Work Environment

The physical therapist will work onsite at our outpatient office in Teaneck, New Jersey, where you will have access to well-equipped therapy spaces as well as residents' apartments for personalized care. Your work schedule will be Monday to Friday from 9 AM to 5 PM, ensuring you can maintain a healthy work-life balance. You will collaborate closely with an interdisciplinary team of nurses, caregivers, and wellness staff, fostering a collegial and team-oriented atmosphere that prioritizes resident safety and engagement.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations for new residents and reassessments for those currently receiving care, ensuring an individualized approach.
• Develop tailored treatment plans aimed at addressing the specific goals and needs of each resident, taking into account their personal circumstances and health status.
• Implement effective treatment strategies that focus on enhancing mobility, balance, strength, fall prevention, and overall functional independence.
• Lead one-on-one therapy sessions either in residents' apartments or in designated onsite therapy areas, creating a comfortable environment that encourages progress.
• Regularly monitor and document patient progress using our electronic medical records (EMR) system, adjusting treatment plans as necessary to meet evolving patient needs.
• Collaborate with nursing and wellness staff to develop and promote safety programs, exercise regimens, and educational sessions on fall prevention.
• Educate residents and their families on effective mobility strategies, the use of assistive devices, and home exercise programs to continue their rehabilitation efforts.

This role primarily focuses on assisting older adults, many of whom may face chronic conditions or functional limitations. Our clients are individuals who require dedicated support in maintaining physical mobility, preventing falls, and achieving independence in daily activities. Qualifications

• A valid New Jersey Physical Therapist License is required.
• Graduation from an accredited Physical Therapy program (Doctor of Physical Therapy, Master of Physical Therapy, or equivalent).
• Current CPR/BLS certification.
• Excellent interpersonal and communication skills, with the ability to build rapport with residents, families, and team members.
• A strong commitment to providing compassionate care, particularly for older adults and individuals with complex health needs.

Preferred Experience

• A minimum of two years of experience in a physical therapy setting, preferably with geriatric or assisted living populations.
• Familiarity with electronic medical record systems and documentation practices in a clinical environment.
• Experience in developing and implementing fall prevention programs or adaptative wellness initiatives would be beneficial.

Compensation and Benefits

The compensation for this position ranges from $100,000 to $120,000 annually, based on experience, along with a comprehensive benefits package.
