Collaborate with the multidisciplinary team to implement and evaluate individualized care plans. Explain diagnoses, treatments, and care instructions to patients and responsible persons. Demonstrate nursing clinical judgment to recognize and report changes in patient condition. Maintain complete and accurate medical records in the patient chart. Participate in continuing education and meet core measure responsibilities related to patient care.
