Vital Motion Physical Therapy Network is seeking a dedicated and skilled Physical Therapist to enhance our rehabilitation services in Pomona, California. Work Environment

You will be working onsite at our rehabilitation facility, collaborating closely with an interdisciplinary team of healthcare professionals, including physicians, nurses, occupational therapists, and speech therapists. Key Responsibilities

• Conduct comprehensive assessments of residents to identify their physical capabilities and specific therapeutic needs.
• Develop and implement individualized treatment plans aimed at improving strength, mobility, and overall functional independence.
• Provide therapeutic interventions such as manual therapy techniques, strength training, balance exercises, and therapeutic modalities to address identified impairments.
• Monitor and document residents' progress meticulously, making adjustments to treatment plans based on their evolving needs and goals.
• Work collaboratively with a multidisciplinary team to ensure holistic care, sharing insights and strategies for comprehensive treatment.
• Educate residents and families on tailored home exercise programs and strategies to maintain and improve physical health outside of therapy sessions.
• Maintain compliance with state and federal regulations, professional standards, and facility policies.

Patient Population / Client Focus

Our patient population primarily consists of geriatric residents who may have various complex medical conditions, including post-surgical rehabilitation, stroke recovery, and chronic illness management. You will play an integral role in crafting and executing treatment plans that align with each resident's unique goals, helping them achieve meaningful progress in their recovery journey.

Qualifications

• A current and valid licensure as a Physical Therapist in the state of California.
• A Bachelor's Degree in Physical Therapy from an accredited institution; advanced degrees (Master's or Doctorate) are highly valued.
• Strong communication, organizational, and interpersonal skills to effectively engage with residents and their families.
• Proven ability to work collaboratively within a team-focused environment, ensuring quality care and coordinated treatment.
• Commitment to continuous professional growth and staying up-to-date with the latest industry practices and standards.

Preferred Experience

• Previous experience in a skilled nursing facility or with an elderly population is highly desirable, enhancing understanding of the complexities of geriatric care.
• Experience with implementing therapy programs tailored to residents with diverse needs will be a plus.

Compensation and Benefits

Vital Motion offers a competitive salary ranging from USD 120,000 to 130,000 per year , commensurate with experience. Additionally, our benefits package includes:
