Key Responsibilities

• Conduct thorough assessments to evaluate clients' physical abilities and health status, using standardized tests and measures.
• Develop, implement, and modify detailed, individualized treatment plans aimed at achieving specific therapy goals related to mobility, strength, and pain management.
• Collaborate with healthcare professionals and family members to streamline treatment approaches and encourage patient engagement in their recovery journey.
• Regularly monitor and document client progress, ensuring that reports are up-to-date and compliant with healthcare regulations.
• Educate clients and their caregivers on safe practices, exercises, and preventative care strategies to support ongoing health and wellness.
• Participate in community outreach programs and wellness initiatives to promote physical health and preventive care across the client base.

Patient Population / Client Focus

Your primary focus will be on providing exceptional care to a diverse clientele, predominantly older adults, who may be facing challenges related to mobility, recovery from surgery, chronic pain, or other physical impairments. This role is pivotal in assisting clients to regain independence and lead fulfilling lives while navigating their unique health challenges.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current and valid Virginia Physical Therapy license in good standing.
• A minimum of 2 years of relevant experience in a physical therapy setting, ideally focusing on geriatrics or home care.
• Strong interpersonal and empathetic communication skills to effectively interact with clients and their families.

Preferred Experience

• Experience using therapy modalities and equipment suitable for home care settings.
• A background in occupational therapy collaboration and knowledge of adaptive techniques for enhancing client mobility.
• Familiarity with electronic health record (EHR) systems for maintaining accurate client documentation.

Compensation and Benefits

We offer a competitive salary range of $90,000 to $100,000 annually, commensurate with experience.
