Key Responsibilities

• Adhere to state practice acts and understand the regulations governing the practice of physical therapy, ensuring compliance while supervising any support staff as needed.
• Conduct thorough examinations and evaluations of patients with functional limitations and disabilities, developing individualized treatment plans tailored to each patient's unique needs.
• Implement evidence-based treatment protocols based on evaluation results and the patient's clinical presentation, continuously adjusting treatment as necessary.
• Collaborate with multidisciplinary healthcare teams, documenting significant changes in patients' conditions and suggesting modifications to therapeutic interventions as required.
• Complete accurate and timely documentation for all patient care activities, including initial evaluations, daily notes, progress summaries, and discharge plans that reflect ongoing treatment and care goals.
• Engage in discharge planning and ensure effective follow-up care for patients, emphasizing continuity of care after leaving our services.
• Participate in facility meetings to discuss patient care initiatives and collaborate on best practices for improving patient access to therapy services.
