Work Environment

This position is based in a home care setting, allowing you to interact closely with clients in their own environments. Key Responsibilities

• Direct comprehensive physical therapy treatment aimed at preventing disabilities, relieving pain, and restoring muscle function, all in alignment with prescribed treatment plans.
• Supervise and mentor Physical Therapy Assistants (PTAs) to ensure the implementation of care plans is effective and consistent with best practices.
• Conduct thorough evaluations of clients' functional abilities using diagnostic techniques to assess muscle, nerve, joint, and overall functional capacity.
• Assess home environments and make tailored recommendations to enhance clients' independence.
• Provide physical therapy services per physician orders, including manual therapy, exercise instruction, and the application of physical agents.
• Fit, evaluate, and adjust prosthetic and orthotic devices, recommending modifications to enhance mobility and quality of life.
• Prepare detailed clinical records, including initial evaluations, care plans, progress notes, and discharge summaries for the client's clinical documentation.
• Collaborate with an interdisciplinary team to shape and evaluate the holistic treatment approach for each client, participating actively in case conferences.
• Educate clients and their families about the use of therapeutic devices and the overall physical therapy program designed for them.
• Ensure timely submission of clinical, payroll documentation, and all required orders as per company policies.
• Maintain accessibility for PTAs during client treatments, as necessary, to support optimal outcomes.
• Undertake additional assignments to align with the company's goals and objectives, contributing to our mission of exceptional patient service.

Patient Population / Client Focus

The physical therapist will primarily work with diverse patient populations who require rehabilitation support in the comfort of their homes. This includes elderly clients in need of assistance after surgeries or chronic illnesses, as well as individuals recovering from injuries, strokes, and various debilitating conditions. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• An active and unblemished Physical Therapist license in the state of Virginia.
• Demonstrated expertise in various manual therapy techniques and effective exercise program development.
• Proficient in using Electronic Medical Record (EMR) systems, particularly Web PT, for precise documentation.
• Strong knowledge of current physical therapy techniques supported by evidence-based research.

Preferred Experience

• Previous experience in home health settings is highly desirable.

• Familiarity with rehabilitation practices tailored to various age groups and diagnoses.

Compensation and Benefits
