Key Responsibilities

• Work closely with the Social Work Manager, conducting thorough evaluations of patients to diagnose and treat a variety of psychosocial issues, applying clinical social work skills effectively.
• Maintain adherence to the National Association of Social Workers (NASW) Code of Ethics and uphold the highest standards of professional practice.
• Implement the clinic's corporate values—respect, honesty, fairness, and excellence—while delivering accessible and high-quality services to clients in our community.
• Conduct comprehensive bio-psychosocial evaluations to identify barriers to treatment, facilitating improved health outcomes and resource utilization for patients.
• Collaborate with medical and support staff to develop individualized care programs, fostering treatment compliance and enhancing patient satisfaction.
• Design and initiate tailored treatment plans utilizing both individual and group therapy modalities, continuously assessing treatment effectiveness and patient progress.
• Document all social work activities in patients' medical records with precision and in accordance with departmental standards.
• Provide educational resources to patients and families about available therapies, fostering understanding and adherence to recommended interventions.
• Identify and assess advocacy needs of patients and coordinate with local community resources to support their social and emotional development.
• Complete timely case-related statistics and reports as needed, thereby contributing to the clinic’s operational effectiveness.
• Engage in ongoing professional development by participating in training relevant to clinical responsibilities.
• Adhere to all departmental policies and procedures to maintain a productive and compliant care environment.

Patient Population / Client Focus

This role primarily serves a diverse clientele, including individuals facing mental health challenges, emotional difficulties, and complex psychosocial issues. You will be pivotal in helping these patients navigate their challenges and achieve a higher level of psychosocial functioning, offering tailored support that resonates with their unique backgrounds and experiences.

Qualifications

• Master’s Degree in Social Work from an accredited institution.
• Current Licensure as a Licensed Clinical Social Worker (LCSW) from the State Board of Social Work Licensure.
• Strong understanding of human development, cultural considerations, and the impact of aging processes in therapeutic settings.
• Minimum of one year of clinical experience, preferably in health or mental health contexts relevant to our patient demographics.

Preferred Experience

• Previous experience in outpatient therapy settings and familiarity with evidence-based treatment approaches.
• Experience in cross-disciplinary collaboration or integrated care practices.

Compensation and Benefits
