Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children in hospice care, focusing on their emotional, social, and developmental needs to develop personalized care plans.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs) tailored to each child's unique situation and needs.

• Monitor outcomes and progress as a collaborative team member, ensuring that all interventions align with the goals set in the child’s IFSP.

• Provide vital support and resources to families, including education on coping strategies and services available to assist them during the hospice journey.
\n
• Adhere to all local, state, and federal regulations to maintain compliance and ensure the highest standard of care.

• Build rapport with patients, families, and team members to foster a supportive and nurturing environment conducive to emotional healing.

• Implement evidence-based practices and strategies within the Natural Learning Environment framework, designing activities that promote the developmental skills of children.

• Offer coaching and training to families, equipping them with skills to support their child's development at home and enhancing their overall family dynamic.

• Collaboratively assess service frequency to match child developmental progress and family needs, adjusting care plans as necessary to optimize outcomes.

Patient Population / Client Focus\n

The primary focus of this role is on children with disabilities or delays in the hospice context and their families. You will work closely with parents and caregivers, understanding their emotional challenges while providing compassionate support. Your expertise will be crucial in guiding families through difficult times, advocating for their children’s needs, and connecting them with appropriate resources for support.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Valid LCSW license issued by the Department of Public Health in Colorado.

• Minimum of one year of relevant experience working with children in early intervention or hospice settings.

Compensation and Benefits\n
